We are located at 414 Old Hard Road, Suite 501, Fleming Island near Fleming Island High School.

From Jacksonville/Orange Park:

Take Park Avenue/ U.S. Hwy 17 South to Village Square Parkway (two traffic lights south of CR 220).  Turn right onto Village Square Parkway and remain on this road until you reach Plantation Center Drive (approx 0.6 miles on your left).  You will see an orange/coral colored orthodontist building on your left.  Turn left on Plantation Center Drive, travel <0.1 miles to Old Hard Road.  Turn left on Old Hard Road, then take your second left into our parking lot.  Once you are in the complex, we are located in the brick building on the right, Suite 501.

From Green Cove Springs: 

Take U.S. Hwy 17 North to Village Square Parkway (this is the second to the last traffic light before you reach CR 220).  Turn left onto Village Square Parkway and remain on this road until you reach Plantation Center Drive (approx 0.6 miles).  You will see an orange/coral colored orthodontist building on your left.  Turn left on Plantation Center Drive, travel <0.1 miles to Old Hard Road.  Turn left on Old Hard Road, then take your second left into our parking lot.  Once you are in the complex, we are located in the brick building on the right, Suite 501.
                                                       

From Middleburg: 

Take Blanding to CR 220.  Travel East on CR 220 until you come to U.S. Hwy 17.  Turn right onto Hwy 17 and travel south to Village Square Parkway (two traffic lights south of CR 220).  Turn right onto Village Square Parkway and remain on this road until you reach Plantation Center Drive (approx 0.6 miles).  You will see an orange/coral colored orthodontist building on your left.  Turn left on Plantation Center Drive, travel <0.1 miles to Old Hard Road.  Turn left on Old Hard Road, then take your second left into our parking lot.  Once you are in the complex, we are located in the brick building on the right, Suite 501.
