We are located in “Village Square” in Fleming Island Plantation, across from Fleming Island High School.

From Jacksonville/Orange Park:

Take Park Avenue/ U.S. Hwy 17 South to Village Square Parkway (one traffic light south of CR 220).  Turn right onto Village Square Parkway and remain on this road until you reach Plantation Center Drive (approx 0.6 miles).  You will see an orange colored orthodontist building on your left.  Turn left on Plantation Center Drive, travel <0.1 miles to Old Hard Road.  Turn left on Old Hard Road, then take your first left (Market Street) into our parking lot.  We are located in the olive green colored building, Suite 57.

From Green Cove Springs: 

Take U.S. Hwy 17 North to Village Square Parkway (this is the last traffic light before you reach CR 220).  Turn left onto Village Square Parkway and remain on this road until you reach Plantation Center Drive (approx 0.6 miles).  You will see an orange colored orthodontist building on your left.  Turn left on Plantation Center Drive, travel <0.1 miles to Old Hard Road.  Turn left on Old Hard Road, then take your first left (Market Street) into our parking lot.  We are located in the olive green colored building, Suite 57.

From Middleburg: 

Take Blanding to CR 220.  Travel East on CR 220 until you come to U.S. Hwy 17.  Turn right onto Hwy 17 and travel south to Village Square Parkway (one traffic light south of CR 220).  Turn right onto Village Square Parkway and remain on this road until you reach Plantation Center Drive (approx 0.6 miles).  You will see an orange colored orthodontist building on your left.  Turn left on Plantation Center Drive, travel <0.1 miles to Old Hard Road.  Turn left on Old Hard Road, then take your first left (Market Street) into our parking lot.  We are located in the olive green colored building, Suite 57.
